The Judicial Branch assumed the responsibility for creating and maintaining a roster of and credentialing psychologists who conduct court-ordered evaluations in child protection cases effective January 1, 2010.

The roster on the website includes the following information:

• Name of the credentialed psychologist
• Business contact information
• Language proficiencies
• Primary and secondary practice interests if provided by the psychologist

As part of the credentialing process, each provider is asked to sign:

• Certification and Authorization Form - PDF
• Acknowledgement Form - PDF
• DCF Child Protective Service Background Check Form(Word Doc)
